• Partially Edentulous both jaws, only present teeth is 11,21,33,43,45
CBCT ANALYSIS cbct showing very Poor quality D4 bone in maxilla, with sinus dropped in left side.
What we did:
• Seven Implants in Upper and six implants in Lower arch placed under local anesthesia without pain Osstem Taper kit, Osstem Las kit and sinuslift kit used in maxilla for direct sinus lift.
Biooss bone graft used. • CBCT and Digitally Guided Implant Surgery • Osstem Implant system used with Bioss bone graft. • Implant placed with good primary stability >30 NCM interim denture with soft liner given.
Prosthetic Phase.
Healing caps placed after 5 months. • Trios 3shape used for digital scan. • CAD CAM Milled MLS Framework • Upper jaw and lower jaw Cement retained MLS PFM (porcelain fused to metal ) prosthesis on Osstem Implants

A 55 year old Male patient with complaints of mobile teeth, difficulty in chewing and unesthetic smile.
On Diagnosis:
Missing Teeth, Mobile upper teeth ,Lower mobile old ceramic bridge.
What we did: (ALL ON SIX IMPLANT)
Immediate extraction and implant placement in both the jaw in single day with marathon 6 hours surgery.
• Six Implants in Upper and six implants in Lower arch placed under local anesthesia without pain
Osstem Taper kit, Osteotomes and cas kit used for indirect sinus lift.
• CBCT and Digitally Guided Implant Surgery
• Osstem Implant system used
• Implant placed with good primary stability >30 NCM
interim denture with soft liner given.
Prosthetic Phase.
-First appointment
Healing caps placed.
Same visit impression taken and abutments milled and temparary PMMA bridge was given.
-Second appointment
pmma removed,
• Trios 3shape used for digital scan.
• CAD CAM Milled MLS Framework
• Upper jaw and Lower jaw Cement retained MLS prosthesis on Osstem Implants
Two visit complete prosthesis finished.

Case of gingival hyperplasia and chronic Periodontitis. This Patient was scared of implants when we started but by the end of treatment the patient was happy and satisfied with the result.
Patient has a history of taking amilodipine since 4-5years and also taking other medications for hypertension
On examination:
Missing Teeth
Mobile Teeth
Generalized attrition
What we did:
• Seven Implants in Upper and six implants in Lower arch with Immediate extraction, currete, implant placement done. Bone quality d4 with paper thin buccal plate.
Indirect sinus lift n bone grafting done left side maxilla, buccal socket wall grafted with Q oss.
Osstem Taper kit, Osstem Cas kit used in maxilla for indirect sinus lift.
Esset bone expansion kit used for poor ridge maxilla.
• CBCT and Digitally Guided Implant Surgery
• Osstem Implant system used with Q oss bone graft.
• Implant placed with good primary stability >30 NCM
interim denture with soft liner given.
Prosthetic Phase.
Healing caps placed after 4 months.

Exposure of pulp hornes and deep class v lesions, fractured 36 We planned: 1.Diagnostic models-photographs , digital preop scan record, 2.Patient education with wax up models, APT for giving test-drive to the patient and for esthetic-functional evaluation 3. Patient advised for extraction of 36, opted for a bridge 4. RCT of 42 43 44 45 46 and 37 35 34 and 12 5. PMMA trial with fully digital workflow. 6. Replication of same in final monolithic and layered zirconia crowns.$

Immediate implant. Patient presented with Pain on $Immediate implant. Patient presented with Pain on chewing. xray RVG shows Horizontal fracture, CBCT confirmed horizontal fracture. Biggest Challenge was to extract apical third of the root atraumatically. Tooth extracted as atraumatically as possible, apical part removed with H File. Curretage of the socket done. Palatal Pilot Drilling done, went Near to the Canine. With help of Sidecut drill direction changed. Final drill verified with the xray. Placed the implant 2-3 mm to the CEJ . I wanted to prevent recession further, so placed the implant on the same day with grafting. 4 by 10 osstem TS III, Buccal side Q oss bone graft placed in jumping distance.
